Patient FAQ


The HCPP primary care physician and your HCPP specialty physician that you see will all know what is going on with your health because they communicate better with each other; you don't have to have the same medical tests done over and over because your results are shared among your health care team; The providers participating in the HCPP will become partners with you in making care decisions.

HCPP may hire people to help check on your care. They may call you after an appointment or a procedure to make sure you understand how to take your medicines or schedule follow-up visits. They will also share information with your doctor to make sure you get the right care.

The HCPP Physicians are located throughout greater Broward and Southern Palm Beach County. These Physicians can be employed by Holy Cross Medical Group or in private practice. Click here to see those HCPP that have joined our network.

Initially, the HCPP Physicians will monitor the following six quality measures for diabetes and preventative care:

  1. HbA1C- hemoglobin A1C for all patients with diabetes mellitus type 1 & 2
  2. Nephropathy Assessment in Diabetic Patients- urine protein screening or medical attention for nephropathy .
  3. LDL-C Screening- for all patients with diabetes mellitus type 1 & 2.
  4. Breast Cancer Screening.
  5. Cervical Cancer Screening.
  6. Colorectal Cancer Screening.

HCPP Physician will have access to your health care information and are able to share that information with one another, they can give you more complete coordinated care. Each of your HCPP physicians will not only know about the health issues that they have treated, they will have a more complete picture of your health through communicating with all of your other HCPP physicians.

HCPP physicians utilize tools and technology to better coordinate and track care. Sometimes the information needed to best coordinate care comes from your insurance company. While HCPP physicians accept many insurances, only certain plans have contracted with HCPP to share the information needed to improve health outcomes.
