What is Diabetes & Prediabetes? 

Diabetes is a disease that occurs when your blood glucose (also known as blood sugar) is too high.  

According to the American Diabetes Association (ADA), in 2018: 

  • 34.2 million Americans had diabetes (10.5% of the US population) 
  • 14.3 million Americans 65 years of age and older have diabetes (26.8%) 
  • 1.5 million Americans are diagnosed with diabetes each year.

Prediabetes is a health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes.  

According to the Centers for Disease Control and Prevention (CDC), in 2018: 

  • 88 million American adults have prediabetes (34.5% of the adult US population) 
  • 24.2 million people aged 65 years or older have prediabetes.

Programs Available

Holy Cross Health is proud to offer the following programs to our patients and community.  

National Diabetes Prevention Program (NDPP) 

The CDC-recognized Diabetes Prevention Lifestyle Change Program focuses on supporting you in your journey to improve your overall health and well-being.  Throughout the year-long program, a trained Lifestyle Coach will help you learn and develop new skills around nutrition, physical activity and stress management. Creating these lasting healthy habits can help to reduce your risk of developing type 2 diabetes and other chronic conditions. 

Program Eligibility 

You may be eligible for the program if you meet the following criteria: 

  • Are 18 years or older AND 
  • Are overweight or obese (BMI ≥25; ≥23 if Asian American) AND 
  • Have no previous diagnosis of type 1 or type 2 diabetes AND 
  • Are not currently pregnant AND 
  • Have one of the following blood test results in the prediabetes range within the past year: 
    • Hemoglobin A1C: 5-7%-6.4% OR 
    • Fasting blood glucose: 100-125 mg/dL OR 
    • Two-Hour glucose: 140-199 mg/dL OR 
  • Have a previous diagnosis of gestational diabetes OR 
  • Have received a result of 5 or higher on the Prediabetes Risk Test

Diabetes Self-Management Education & Support/Training Program (DSMES/T) 

DSMES/T includes individual and group education.  During a one-on-one session with a Certified Diabetes Care & Education Specialist, you will create a plan and set goals that fit your needs and lifestyle.  In the group setting, you will learn ways to self-manage your diabetes while finding other people who have the same concerns and challenges as you.  Group class topics include healthy eating, being active, blood sugar monitoring, taking medication, problem solving, reducing risks and healthy coping.  
 

Program Eligibility 

You may be eligible for the program if you meet the following criteria: 

  • Diagnosis of diabetes 
  • Have one of the following blood test results: 
    • Hemoglobin A1C: ≥ 6.5% 
    • Fasting blood glucose: ≥ 126 mg/dL on 2 different occasions 
    • Oral glucose tolerance test: ≥ 200 mg/dL on 2 different occasions 
    • Random glucose test: > 200 mg/dL with symptoms 
  • Referral from your healthcare provider managing your diabetes 
    • Find an Endocrinologist here
