Holy Cross Health has partnered with Exercise is Medicine® (EIM) to make physical activity assessment and promotion a standard in clinical care.

Holy Cross medical providers will refer patients for those at risk for chronic disease to evidence-based exercise programs and qualified exercise professionals.

We are committed to the belief that physical activity promotes optimal health and is integral in the prevention and treatment of many medical conditions.

Program Overview 

Implementation of a clinical program to include 

  • 12-week Exercise program
  • Nutrition support coaching
  • Healthy lifestyle education
  • Behavior change interventions

Desired outcomes of the program 

  • Patients who are educated and motivated, empowered to continue participating in exercise programs, make healthy lifestyle choices
  • Increase readiness for change and self-efficacy to perform physical activity resulting in improved healthy behaviors and quality of life
  • Improve patient/community population health 

How It Works 

Visit your Holy Cross provider for assessment

  • Identify patients who are not meeting the physical activity recommendations of 150 minutes per week
  • Identify patients who have been diagnosed with/or at risk for obesity, diabetes, hypertension, hyperlipidemia
  • Refer to Holy Cross Exercise is Medicine® program
