Holy Cross Hospital specializes in providing a full range of home health services. The primary objective is to provide an extensive range of care in the home according to individual needs, encouraging maximum rehabilitation and independence. Our staff, specially trained for in-home care, work directly with the patient's doctors and family to establish a comprehensive recovery plan. Our staff will determine a patient´s full range of home health benefits and order appropriate equipment and supplies.

As a Medicare Certified Agency with exceptional clinical outcomes, Holy Cross Home Health continues to be a leader in home health services. In 2012, Holy Cross Home Health received back-to-back deficiency-free surveys: from The Joint Commission (TJC) in May and Florida's Agency for Health Care Administration (AHCA) in November.

We are fully accredited by the Joint Commission, with excellent results. When Outcomes Concept Systems (OCS) announced its "2006 Home Care Elite" recognizing the most successful home care providers in the U.S. Holy Cross Home Health was named in the top 25% of providers nationwide. This rating, based on performance measures in quality improvement and financial outcomes, has been earned by Holy Cross Home Health in 2007, 2008 and 2010, as well.

Home Health Services

The home care team, composed of RNs, LPNs, Physical Therapists, Physical Therapy Assistants, Occupational Therapists, Speech Therapists, Medical Social Workers and Home Health Aides, works with the patient´s physician to provide the following services:

  • Disease/Post-surgical management

  • Wound Care/Catheter Care and changes
  • Infusion / Enteral/Parenteral therapies

  • Rehabilitation: PT, OT, ST
  • Home Safety assessments and safety equipment planning and instruction

However, it is important to consider that one out of three older adults (65 years old and older) falls each year, and falls are the leading cause of both fatal and nonfatal injuries in older adults. In addition to fall prevention, another important health issue in older adults is preventing adverse drug events. As people age, they typically take more medications, so the risk of adverse events may increase. It is vital to ensure that older adults are remembering to take their medications and that they are taking the right medications. Adverse drug events cause over 700,000 emergency department visits each year, and nearly 120,000 patients each year need to be hospitalized for further treatment after emergency visits for adverse drug events. Older adults are twice as likely as others to come to the ER for adverse drug events and almost seven times more likely to be hospitalized after such an emergency visit (Centers for Disease Control and Prevention).

Guardian Medical Alert

Guardian Monitoring service gives you the confidence and freedom to live on your own, because there´s always someone standing by to help whenever you need it, 24-hours-a-day, 7-days-a-week, all at the press of a button. Our technologies to promote independence include:

• Water resistent pendant with a medical alert button
• Dials into Guardian Call Center at the push of the button
• Guardian operator places call on caller's behalf to appropirate person (facility on-call, family member, neighbor and/or 911)

Automated Medication Dispensing Units

Technologies are available to improve medication adherence so that you or your loved one take(s) the right medications at the right time, everytime.

Guardian Large Automatic Medication Dispenser

The larger system announces a reminder, "Time for your medicine, press button to dispense." Once you push the large blue button, the right dose of medication is delivered. If you miss any doses, your caregiver is notified. The unit is also kept safe and secure - it is locked to prevent from tampering. This unit requires to be plugged into a telephone line and a power outlet, and it contains back-up battery in case of power outages to make sure the service is uninterrupted. It also notifies you when the batteries require replacing, and our technicians can be dispatched to your home to replace them for you. In addition, technicians come to your home for the initial set-up of the equipment to make sure that it is in working order.

Guardian MedMinder
The smaller, portable device - MedMinder - sounds an alert, and the appropriate compartment illuminates when it is time to take your medication, making it ideal for low hearing and low vision subscribers. If the medication is not taken when prompted, or the wrong compartment is opened, a caregiver is notified. The reminders are easily programmable. It holds upto four doses a day and seven days of medications.

Telehealth

The first personal, in-home, daily information link between you, your physician and our home health agency. This easy-to-use appliance helps you to provide important information to your home health nurse on a daily basis from the comfort of your home. Now you can take a more active role in your well-being, and enjoy greater peace of mind knowing that your caregivers have up-to-date information on your health every day.

Care Management

Our objective is to respect and value the comfort and security of the home setting for the sick and elderly. We work with the patient, doctor and family in establishing a plan of care that meets the patient´s individual needs. Each patient is closely supervised by a Registered Nurse throughout their care for:

  • Health care and medications management
  • Personal care needs
  • Household management
  • Shopping and meal preparation
  • Coordination of services designed to help the patient remain in their home as independently as possible

Holy Cross Private Duty

Holy Cross Private Duty is committed to providing quality and affordable home care services. To us, nothing is more important than helping our patients live full and dignified lives with the greatest degree of independence. Holy Cross Private Duty specializes in providing a full range of home health services. We offer private duty care in the hospital and in the home setting at hourly or 24-hour live-in rates. At Holy Cross Private Duty, the patient is our primary focus.

Holy Cross Private Duty allows patients to receive care while remaining in the comfort of their homes. We provide professional, high quality home care services through carefully selected nurses and health care professionals. We hire our caregivers based on their professionalism, enthusiasm and compassion for patients. After extensive screening, we bond and insure each one. Holy Cross Private Duty employs trained Home Health Aid (HHA)/Certified Nursing Assistant (CNA) aides to provide companionship and maintain a safe environment.
