Primary care providers are often the first contact you have with a healthcare system. Our primary care physicians are here to keep you healthy and help manage the care of any chronic conditions you might have. The more we keep you healthy, the lower your healthcare costs. In fact, when people do not have access to a primary care provider, they end up in emergency rooms and are admitted to hospitals more often. We are a Level 3 Certified Patient Centered Medical Home, and we aim to be your trusted health partner for life.
A Patient Centered Medical Home (PCMH):
• Builds a partnership with its patients: a clinical summary with physician recommendations is provided, as well as informational materials customized to their individual needs so that they can be an active participant in their care at home. While conforming to HIPAA regulations, caregivers - including family members - are enlisted to help. Patients are also included in decisions about which treatment approach is right for them.
• Ensures accessibility: appointment times are increased to allow for more comprehensive patient visits. Additionally, several slots are left open for patients who call in and need to be seen that day or the next.
• Provides care coordination: the appointment schedule is reviewed well in advance to identify patients with chronic conditions. As appointments are scheduled, a nurse reviews the patient's records to determine if tests were ordered and completed, and if not, the nurse follows up with the patient prior to the appointment.
• Offers the convenience of electronic health records (EHR): Holy Cross Medical Group offices are equipped with various robust EHR tools, including a disease registry that collects data from multiple providers allowing us to track patients who receive care elsewhere, thereby ensuring that you receive the care you need based on clinical practice guidelines. We are able to track and monitor each patient's progress.
All of the above objectives are designed to achieve a single goal: keeping the patient at front and center.
There are currently five Level 3 Certified Patient-Centered Medical Homes in the Holy Cross Medical Group, and we plan to create several additional PCMH facilities in the future.
