FORT LAUDERDALE, Fla. – For the fourth consecutive year, Holy Cross Hospital in Fort Lauderdale has received the American Heart Association/American Stroke Association’s Get With The Guidelines®-Stroke Gold Plus Quality Achievement Award in recognition of the non-profit hospital’s commitment and success in implementing excellent care for stroke patients, according to evidence-based guidelines.
To receive the award, Holy Cross achieved an 85 percent or higher adherence to all Get With The Guidelines-Stroke Quality Achievement indicators for 12 consecutive months and achieved 75 percent or higher compliance with five of eight Get With The Guidelines-Stroke Quality Measures, which are reporting initiatives to measure quality of care. In addition to the Get With The Guideline-Stroke award, Holy Cross Hospital has also been recognized as a recipient of the association’s Target: Stroke Honor Roll for improving stroke care.
Through its Comprehensive Stroke Center, Holy Cross provides round-the-clock personnel, infrastructure, expertise and programs to diagnose and treat stroke and transient ischemic attack patients who require a high degree of medical, surgical and endovascular care, specialized tests or interventional treatments. The Joint Commission certified advanced Primary Stroke Center is also distinguished by The Agency for Health Care Administration as a Comprehensive Stroke Center.
The American Heart Association also recognized Holy Cross Hospital with the Get With The Guidelines®-Heart Failure Bronze Quality Achievement Award for achieving an 85 percent or higher adherence during the fourth quarter of 2012 to all Get With The Guidelines® Heart Failure Achievement indicators to improve quality of patient care and outcomes. Holy Cross Hospital reduced hospitalizations and improved heart failure patient care by opening a Congestive Heart Failure Clinic in 2011. The Clinic provides intensive personalized therapy and develops individual programs that educate patients about heart failure, optimize chronic medications and help patients manage their disease. The Heart Failure Clinic’s 30-day readmission rate in 2012 was four percent.
Get With The Guidelines®-Stroke and Get With The Guidelines®-Heart Failure are in-hospital programs for improving care by promoting consistent adherence to the latest scientific treatment guidelines.
