Home Health Services
Holy Cross Hospital specializes in providing a full range of home health services. The primary objective is to provide an extensive range of care in the home according to individual needs, encouraging maximum rehabilitation and independence. Our staff, specially trained for in-home care, work directly with the patient's doctors and family to establish a comprehensive recovery plan. Our staff will determine a patient's full range of home health benefits and order appropriate equipment and supplies.
Scope of Services
We provide experienced health care professionals under the supervision of your physician. The home care team may include:

- Licensed Practical Nurse
- Registered Physical Therapist
- Registered Occupational Therapist
- Certified Speech Therapist
- Medical Social Worker
- Home Health Aide
RN's and LPN's may provide instruction in your medications, disease processes and prevention. Additional skills include: wound care, cardiac rehabilitation, orthopedic rehabilitation, infusion therapy, maternal/child care, laboratory services and much more. Personal care and support services are provided by a Home Health Aide.

TELEHEALTH
Health Buddy is a remote disease management monitoring solution that assesses patient knowledge of their condition and assists in enhancing patient knowledge. It also collects vital signs, symptoms and behavioral information from patients. Combined with the visits from our highly skilled clinicians, gaps in patient monitoring are eliminated. This customized care delivery results in better long term disease management, fewer crisis situations and improved patient outcomes. This program is provided to our home health patients free of charge.
