Holy Cross Hospital specializes in providing a full range of home health services. The primary objective is to provide an extensive range of care in the home according to individual needs, encouraging maximum rehabilitation and independence. Our staff, specially trained for in-home care, work directly with the patient's doctors and family to establish a comprehensive recovery plan. Our staff will determine a patient´s full range of home health benefits and order appropriate equipment and supplies.
As a Medicare Certified Agency with exceptional clinical outcomes, Holy Cross Home Health continues to be a leader in home health services. For the past 15 years, state and federal surveys by the Agency for Health Care Administration (AHCA) have found us to be deficiency-free.
We are fully accredited by the Joint Commission, with excellent results. When Outcomes Concept Systems (OCS) announced its "2006 Home Care Elite" recognizing the most successful home care providers in the U.S. Holy Cross Home Health was named in the top 25% of providers nationwide. This rating is based on performance measures in quality improvement and financial outcomes.
The home care team, composed of RNs, LPNs, Physical Therapists, Physical Therapy Assistants, Occupational Therapists, Speech Therapists, Medical Social Workers and Home Health Aides, works with the patient´s physician to provide the following services:
- Disease/Post-surgical management

- Wound Care/Catheter Care and changes
- Infusion
- Enteral/Parenteral therapies
- Pain Management
- Nutritional Guidance
- Rehabilitation: PT, OT, ST
- Home Safety assessments and safety equipment planning and instruction

Our objective is to respect and value the comfort and security of the home setting for the sick and elderly. We work with the patient, doctor and family in establishing a plan of care that meets the patient´s individual needs. Each patient is closely supervised by a Registered Nurse throughout their care for:
- Health care and medications management

- Shopping and meal preparation
- Coordination of services designed to help the patient remain in the own home as independently as possible
