Community Outreach
The Community Outreach Team practices in a variety of settings, including elementary schools, parishes, community agencies, parks, temples, child daycare facilities, homeless shelters and community centers.
In their quest to keep our community healthy, the Community Outreach Team works in partnership with the local organization, community groups and other healthcare professionals. Community partnerships are particularly important because they not only translate into a better understanding of local health needs, but also facilitate the development of cooperative prevention strategies and improved outcomes. These efforts also remove any duplication of efforts or services by local providers.
One of the most important tasks our Community Outreach Team has is to link individual clients and families with the support and services found at the community level that meet their needs through care coordination. The professional and personal connections, and trust, built with each client is key to assuring their continued participation in care and treatment.
