The Centers for Disease Control and Prevention recommends performing a 2-step tuberculin skin test (TST) on all newly employed Health Care Workers who have an initial negative Tuberculin Skin Test result at the time of hire and who have no documented negative Tuberculin Skin Test result during the past 12 months. The purpose of the 2-step Tuberculin Skin Test is to identify Health Care Workers who have had M. tuberculosis infection but whose ability to react to purified protein derivative has waned with time. Subsequent re-challenge with purified protein derivative can boost the hypersensitivity reaction, resulting in a positive TST result. Two-step TST can be used to reduce the likelihood that a boosted reaction is incorrectly identified as a conversion due to recent infection.
